Grace ADHC Center is a licensed day health care program by California Department of Public Health that provides a combination of medical, social and therapy services to adults who have difficulty functioning in their own homes.

Our highly qualified staff can provide a structured day program which includes nursing, meals, transportation, social services and restorative therapies such as physical, occupational and speech therapies, in a warm, caring and secure environment.

Grace ADHC provides medical, rehabilitative and social services to elderly persons and other adults with functional impairment, either physical or mental, for the purpose of restoring or maintaining optimal capacity for self care. Grace ADHC will provide top quality services through an individual plan of care and targets rapid growing number of adults who are at the institutional level of care or at risk of institutional placement. Grace ADHC is a Medi-Cal benefit program and is administered by California Department of Aging (CDA) and licensed by the California Department of Public Health as a health care facility. Grace ADHC is an approved Community Based Adult Services (CBAS) provider.

Our Center’s competitive edge is the quality of the licensed professional staff as well as the modern facility to provide the best possible care. Our diverse staff members will care about the health, safety, and well being of each of the participants. Our team will include multidisciplinary professionals such as Registered Nurses, Speech Therapists, Occupational Therapists, Physical Therapists, Dietitian, Social Workers, and Activities Coordinators.

The entire program cost may be covered by Medi-Cal, Managed Health Plan, VA, or LTC Insurances. A sliding scale fee is charged to participants who are not covered by Medi-Cal (Private Pay).
