Grace Adult Day Health Care provides medical, rehabilitative and social services to elderly persons and other adults with functional impairment, either physical or mental, for the purpose of restoring or maintaining optimal capacity for self care. We provide top quality services through an individual plan of care and target a rapid growing number of adults who are at the institutional level of care or at risk of institutional placement.

Public Service Announcement
Grace Adult Day Health Care Center announces the sponsorship of the Adult Day Care Food Program.
Meals will be available at no separate charge to people enrolled at the Grace center. The United States Department of Agriculture (USDA) and the California Department of Education’s Nutrition Services Division (NSD) prohibit discrimination in all Their programs and activities on the bases of race, color, national origin, gender, religion, age, or disability.
